I have a 6 month old daughter, she has no father listed on her birthcirtificate. However, she does have his last name.

Her father left 3 days before I had her. he was on probation in Ga & they made him go back. We talked over the phone for about a month or two after i had her, the last i heard he was in jail.

He chose to be irresponsible & make choices to keep her out of his life. I do not want her to have anything to do with him, considering the lifestyle he chose to lead.

I really wanted to know how i would go about getting her last name changed to mine without him signing anything. I know he would never be willing to do so, because he also has a 5 yr old that he barely had anything to do with & refused to sign over anything.
