I took out a short term disability policy with afflact At the moment I'm short term disabled at this point. The problem was a bad reaction to an antidepressant. After reveiwing the policy it appears as through they do not cover disability to depression. The question that I have is that the questionaire filled for the policy was completed by someone other than myself and my name was forged. What can I do about this.
