Unhappy confounded filing & paperwork

I'm filing a breach of contract in small claims or county civil (I'd like to go higher but don't think I can here in Florida w/o an atty) and the problem isn't one of law directly but of administrative procedure and formatting. Some of the forms the clerk have are partially mad lib style, what type of thing goes in this line and that, but not comepletely, and of course, they're prevented from advising.

The claim is against a cell phone company not incorp'ed in Florida so I know to list their resident agent and who that is, but not how it all goes on the paperwork. What combination of phone co., their address & officers, and the res. agent business name, address & representatives? http://sunbiz.org/pdf/00525960.pdf And there's another manager listed at www.manta.com.

On the General Motion form, there's "provided by U.S. Mail to Defendant/Respondent, _________, at ____________,..." Is that "at" a time of day, when you mailed it? Or maybe address, but it doesn't provide much room. http://www.clerk.leon.fl.us/sections...ral_motion.pdf

What's the basic paperwork I have to file (before deadline, which is imminent) to start proceedings, then add on like records and evidence, transcriptions, subpoenas, etc.


I'm a college educated person, but geez, could somebody yahoo or google chat/IM with me and walk me thru?!
